THE DEADLINE TO SUBMIT THE GALA QUESTIONNAIRE WAS JANUARY 1, 2025 AT 11:59PM. THE WINDOW TO SUBMIT A QUESTIONNAIRE IS NOW OVER AND DUE THE VOLUME OF APPLICANTS, GALA DOES NOT ACCEPT LATE SUBMISSIONS. PLEASE DO NOT MAKE AN ACCOUNT OR SUBMIT A LATE QUESTIONNAIRE.
IMPORTANT LAUSD UPDATES AS OF DECEMBER 5, 2024: The GALA supplemental questionnaire originally opened on October 1, 2024. Shortly after, on October 4, 2024, LAUSD announced changes to the procedures for “Unique Educational Pathway” schools, initially removing all supplemental questionnaires. LAUSD reconsidered its decision for the 2025-2026 admission cycle and as of December 4, 2024, LAUSD approved a revised GALA questionnaire. We sincerely apologize for any confusion or inconvenience this situation may have caused.
The GALA questionnaire is available starting December 5, 2024 with a deadline of January 1, 2025. The questionnaire is only for families that completed the LAUSD Choices application for GALA by November 15, 2024.
- "This is my first time here." If you have not made a GALA questionnaire account, click "Create an Account" below to fill out the questionnaire. For the account holder information, do NOT use the student name or student email address. The system will send you an email to verify your information.
- "I filled out a questionnaire from a previous year." OR "I made a questionnaire account earlier this year." Click "Sign In" below and use your same login information.
- "I completed the GALA questionnaire for 2025-2026." If you already completed the questionnaire when they opened beginning of October, you do NOT need to complete a new questionnaire. You can sign in to check your status.
